Add one mandatory checkbox for the processing of personal data and show your cancellation and deposit terms before the client confirms. Health questions and treatment consent belong at the consultation, not on the quick online form, and marketing permission must always be a separate, optional choice.

TL;DR:

Six questions cover almost every appointment: name, phone number, service, stylist, date and time, and an optional comment.

Under the GDPR data minimisation principle, every field needs a clear reason; if you can’t say why you ask, remove the field.

Allergies, medication and pregnancy can be data concerning health, a special category under GDPR, so ask about them at the consultation and keep the notes where few people can see them.

Keep booking terms, the personal-data checkbox and marketing permission apart, and never pre-tick a box.

Show cancellation and deposit terms before the client confirms and repeat them in the confirmation message.

The six questions every booking form needs

A booking form only works if it captures what the visit needs without losing the client before they press “confirm”. Get the core fields wrong and you end up chasing people for a phone number after they have booked, or blocking a short slot for what turns out to be a full colour correction. These six questions belong on every form.

Who are you? A first name, plus a last name if you need it to tell clients with the same name apart.

How can we reach you? A phone number works best for reminders and last-minute changes; email suits confirmations and receipts. Ask for one reliable channel and make the second optional.

Which service? Each service should carry its own duration and price, so the calendar blocks the right amount of time and the client knows what they will pay before they arrive.

With whom? Let clients choose a specific stylist or “any available”, and check that person’s own calendar rather than general opening hours.

When? Show only slots that are genuinely free. Rejecting a time after the client has filled in everything else is one of the fastest ways to lose them.

Anything we should know? One short, optional comment field catches the unusual: a first visit, an event date, a child coming along.

Check the format of the phone number and email as the client types, not after submission, so a typo doesn’t mean they never receive their reminder.

A few optional fields can earn their place, as long as you know what you will do with the answer:

How the client heard about you, if you will actually use the answers to judge your marketing.

Practical details that change the time you need, such as removal of existing gel nails.

Anything else usually belongs at the consultation, not on the booking form.

What to leave out: data minimisation and health questions

The most common mistake is one long form that asks every client about allergies, medication and pregnancy, whether they are booking a fringe trim or a full head of bleach. It loses clients at the last step, and it collects more than you need.

GDPR sets the rule in Article 5(1)(c): personal data must be “adequate, relevant and limited to what is necessary” for the purpose you collect it for. A question you can’t link to the appointment fails that test.

Health details need even more care. GDPR defines data concerning health as personal data that reveals information about a person’s physical or mental health status (Article 4(15)). Article 9 makes it a special category: processing it is prohibited unless one of the listed exceptions applies, for example the person’s explicit consent for one or more specified purposes. Answers about allergies, medication, skin conditions or pregnancy can fall into this category.

In practice, that leads to four steps:

Keep the online form free of health questions. The appointment itself rarely needs them.

Ask at the consultation or patch test. For colour, chemical straightening, lash lifts and other skin-contact services, go through allergies, medication, recent skin treatments, pregnancy or breastfeeding and previous reactions in person, with a separate treatment consent form.

Record the patch test. Note the product, where on the body it was applied, the date and time, the result and who carried it out. Follow the product manufacturer’s instructions on when a test is needed.

Reconfirm at each visit. One question, “Has anything changed since your last visit?”, catches new medication or reactions without repeating the whole questionnaire.

If your booking tool can show extra questions only for selected services, use that for practical details such as removal of old gel. For health information, a conversation at the chair is usually the safer route, and the kinder one.

Consent on the booking form: keep the parts apart

Three different things often end up at the bottom of one form. Treat them separately rather than as one “I agree to everything” box.

Booking terms. Deposits, the cancellation window and the late-arrival rule are part of the deal you make with the client. Show them clearly before the client confirms.

Personal data. A checkbox with a link to your privacy notice that explains how you use the client’s details for the appointment.

Marketing. Permission to send offers and newsletters. If you ask for it on the form, make it a separate, optional, unticked box; if your form can’t do that, ask at the first visit and keep a record.

The GDPR wording backs this up. Consent must be a freely given, specific, informed and unambiguous indication of the person’s wishes (Article 4(11)), and Recital 32 says that silence, pre-ticked boxes or inactivity do not count as consent. Where a consent request sits next to other matters, it must be clearly distinguishable from them (Article 7(2)), and the client must be able to withdraw consent as easily as they gave it (Article 7(3)). Article 7(4) adds that making a service conditional on consent to processing the service doesn’t need weighs against that consent being freely given, so “no newsletter, no appointment” doesn’t work.

Not everything on the form needs consent. Processing that is necessary to perform a contract with the client, or to take steps at their request before entering into one, has its own lawful basis under Article 6(1)(b), which usually covers the name, phone number and appointment details. Rules on email and SMS marketing also come from national laws based on the ePrivacy Directive, and they differ between countries, so check yours before you send the first campaign.

Your booking software usually processes client data on your behalf, which makes it a processor, so you need a written contract with the provider (electronic form counts, Article 28(9)). Article 28(3) lists what that contract must cover, including that the processor acts only on your documented instructions, keeps the data confidential, secures it, uses sub-processors only under agreed conditions, helps you answer requests from clients exercising their rights and meet your security obligations, and deletes or returns the data when the service ends. Ask your provider where the data is hosted and whether it leaves the EEA.

Patch-test records and health notes deserve tighter access than the general appointment calendar. Limit who on your team can see them, and set a retention period: the storage limitation principle in Article 5(1)(e) says data should be kept in identifiable form no longer than necessary for the purpose.

Pro Tip: Audit your form today with one question: if a client disputed a late-cancellation charge, could you show that your terms were in front of them before they booked, separately from any marketing consent? If not, that’s your first fix.

Cancellation and deposit wording that holds up

Vague cancellation policies protect no one. Specific ones, shown before the client confirms, do. Here is example wording to adapt; the numbers in brackets are placeholders for your own rules, not a standard.

Cancellation window: “You can cancel or move your appointment up to [24] hours before it starts. Your deposit then stays on your client balance for your next visit, or we refund it on request.”

Late cancellation and no-shows: “If you cancel later than that, or don’t come without letting us know, we keep the deposit.”

Late arrival: “We hold your appointment for [10] minutes. After that we may need to shorten the service or move it to another time.”

How much you may keep after a late cancellation, and how the terms must be presented, depends on national consumer law, so check yours before you set the amounts.

Show the policy in three places: on your booking page, in the confirmation message the client receives right after booking, and at reception.

Three ways to secure a slot cover most situations, and you don’t need all three for every service:

A deposit for most services: low enough not to put genuine clients off, high enough to make a no-show matter. A fixed amount is often easier to understand at a glance than a percentage.

Full prepayment for high-demand slots or first visits, where you have no history with the client yet.

A card guarantee for long or high-value appointments such as colour corrections, extensions or bridal parties: the client saves a card when booking, and your policy protects the slot without an upfront payment.

Whichever you choose, say so on the booking form itself. A client who doesn’t expect to enter card details halfway through will often abandon the form. Put the payment step and the cancellation wording next to each other so they read as one rule.

Cutting friction without cutting corners

A booking form that takes three minutes to complete loses clients before they finish it. Four habits keep it short and still careful.

Put the six core questions first and remove every field you can’t justify.

Add a short line next to any personal question explaining why you ask, and link your privacy notice rather than pasting it in full.

Design for phones: one column, a calendar with clearly separated tap targets, and buttons big enough for a thumb.

Pair the confirmation with an automatic reminder, because reminders and deposits work best together.

Pro Tip: Test your own form on a phone with one hand, standing up, the way many clients actually book. If you fumble a tap target, they will too.

Templates for new clients, colour and nails

New client, online. The six questions translate into about seven fields: name, phone number, optional email, service, stylist or “any available”, date and time, and an optional comment. Below them sit the personal-data checkbox with a link to your privacy notice and, if your form supports it, a separate unticked marketing opt-in.

Colour, at the consultation. Go through these in person before the first colour service:

Known allergies or previous reactions to hair colour.

Current medication or recent skin treatments.

Patch-test record: product, site, date, time, result and who carried it out.

Nails, online. Nail services need practical extras that change the time you book: whether existing gel or acrylic has to be removed, design complexity (plain, French or nail art) and whether the client wants extra time for intricate work. These are not health questions, so they can sit on the online form as separate services or a short optional question.

Use these as an online flow where your tool allows it, or print them as a one-page form for walk-ins and consultations.

Client history belongs in the client card

A form that only asks about today misses what you learned at every previous visit: the last colour formula, the preferred nail shape, a note about sensitive skin. Rather than asking returning clients for their full history again, keep it in the client card and ask one reconfirmation question when they book or when they arrive.

For new clients, a short history at the first consultation is worth it: previous experience with the service, past reactions, and current hair or skin condition where it matters for the treatment. Record what the treatment needs, not everything the client mentions.

That is the practical argument for booking software with client cards rather than a bare calendar: the history stays, the form stays short, and nobody has to ask the same question for the fifth time. Our guide to the client card lists what to record for cosmetology, lashes and nails.

Reminders: ask only what you can honour

A “text, email or both” question only helps if your system can send each client’s reminders through the channel they pick. If every client gets the same automatic reminders, tell them which channel you use instead of asking.

Timing matters as much as the channel. A reminder about a day before the appointment catches most genuine forgetting without feeling like nagging, and automating it removes the morning call list from reception.

Reminders also support your cancellation policy: when a client was reminded and still didn’t come, it is hard to argue they were never told.

The one change that fixes most booking forms

Most booking forms fail because they bundle things that should be separate. Fix that first: one personal-data checkbox, terms shown before confirmation, marketing permission on its own, and health questions moved to the consultation. Then brief your team on the new flow and check every week where clients drop off while the change is still fresh.

Setting up the booking form in Altegio

Altegio online booking is used by appointment-based businesses – beauty, wellness, clinics, auto, fitness – and the advice above maps onto its settings like this:

Service, team member, time. The client chooses a service, a team member and a free slot. With the Any professional mode, the client picks the service and time and the system assigns an available team member; you can also give each team member a personal booking link. In the online booking settings you decide which time slots clients see and how far in advance they can cancel or reschedule online.

Contact details. In Online Booking > Settings > Customize client data fields you can make the email and comment fields required or hide them, and rename the comment field (up to 60 characters; the default is “Appointment comment”). A label such as “Practical notes for your visit (no medical details, please)” fits and steers clients away from sharing health information. Last and middle names can be made required once you show those fields in Settings > Digital Schedule Settings. SMS verification of the client’s phone number is available when your Location has an SMS provider or chat-bot connected.

Extra questions. Custom fields for appointments or clients (text, number, list of values or date) are created in Settings > Custom Fields in the Chain interface or in the online booking settings of a Location. They can be shown in the widget and set as mandatory or optional. A field shown in the widget appears in all widgets of the Locations you select, not for one service only, so phrase it so it makes sense for every client.

Sensitive notes. A custom client field that isn’t shown in the widget is visible only to team members with access to the Location, and viewing or editing client custom fields is a separate access right. That suits patch-test records and sensitivity notes kept in the client card in your client base.

Consent. On the Privacy policy tab of the same settings you switch on the checkbox for consent to the processing of personal data and add your text in each language the widget offers; a client can’t book without ticking it, and you can limit which team members may edit the text. There is no separate marketing-consent checkbox in the widget, so collect marketing permission another way and record it; if a client opts out, exclude them by hand in their client card.

Cookies. For visitors in the EU, the booking form blocks analytics and advertising cookies until the visitor consents.

Deposits and card guarantee. Online payments run through a connected payment provider. Prepayment can be 100% or any percentage; the deposit mode works with any provider available in your region, while the card guarantee works only with Adyen in EU countries. A deposit kept after a timely cancellation stays on the client’s balance and can be refunded manually. Details are on the no-show protection page.

Policy in the confirmation. Automatic notification templates can’t be fully rewritten, but you can add your own text, image or link to them, for example a short cancellation policy. Email notifications are free, and the reminder can be sent up to 24 hours before the appointment; SMS and messenger reminders go through a connected provider.

The price depends on the number of team members and the billing period, as shown on the pricing page. To try the setup on your own services, start a 7-day free trial.

FAQ

What questions should a salon booking form ask?

Six: the client’s name, a phone number (email optional), the service, a preferred stylist or “any available”, the date and time, and an optional comment. Add a mandatory checkbox for the processing of personal data and show your cancellation terms before the client confirms.

Can I ask about allergies on an online booking form?

You can, but you usually shouldn’t. Answers about allergies and medication can be data concerning health, a special category under Article 9 of GDPR that may only be processed under one of its exceptions, such as explicit consent. Ask at the consultation or patch test instead, and keep the notes where only the team members who need them can see them.

Do I need a separate marketing consent checkbox?

Yes, if you want to send marketing based on consent. It must be optional, unticked and clearly separate from the booking terms and the personal-data checkbox, because GDPR consent has to be specific and pre-ticked boxes are not consent. National rules on email and SMS marketing add their own requirements.

What should clients avoid doing before a salon appointment?

Before a colour consultation, avoid heavy styling products and skip at-home chemical treatments in the days before the visit, since both can interfere with a patch test or the treatment itself.

How do I word a haircut booking request?

Name the service, give your availability and mention any stylist preference. An online booking form turns this into a confirmed slot straight away, without back-and-forth messages.

Does Altegio support GDPR-compliant booking forms?

Altegio gives you the building blocks: a mandatory checkbox for consent to the processing of personal data with your own text in each language, contact fields you can make required or hide, custom fields with their own access rights, and cookie blocking for EU visitors until they consent. Whether your form complies depends on how you set it up and what you ask, so keep it short and move health questions to the consultation.

Read next

Client card – cosmetology, lashes, nails: what to record and a template to download

Reminders and deposits: how to reduce no-shows

Protect your most expensive slots: payment policies, card guarantee and Adyen

Google Tag Manager now runs inside Altegio online booking: 11 ways to use it

This article is general information, not legal advice.
