What is FastSign?<\/strong><\/h2>\n\n\n\n

FastSign<\/strong><\/a> (also shown as QR Forms<\/strong>) is a marketplace integration for Altegio, developed by Mapty.ai. It lets you create branded intake forms and share them with clients by link or QR code. When a client fills out a form, the information is written straight into their client card<\/strong> or appointment<\/strong> in Altegio \u2014 so your records stay up to date without anyone re-typing anything.<\/p>\n\n\n\n

It’s designed for Altegio locations that want a fast, paperless way to gather client details, consents, and questionnaire answers before or during a visit.<\/p>\n\n\n\n

What you can use it for<\/strong><\/h2>\n\n\n\n

  • Client intake<\/strong> \u2014 collect name, contact details, date of birth, and other profile fields for new clients.<\/li>\n\n\n\n

    How it works<\/strong><\/h2>\n\n\n\n

    The integration follows a simple, automated flow:<\/p>\n\n\n\n

    1. A new client or appointment is created<\/strong> in Altegio.<\/li>\n\n\n\n
    2. FastSign adds the form link to the notes<\/strong> of that client or appointment.<\/li>\n\n\n\n
    3. The link is sent to the client<\/strong> \u2014 automatically (e.g. via WhatsApp) or shared manually by your team.<\/li>\n\n\n\n
    4. The client completes the form<\/strong> from their phone.<\/li>\n\n\n\n
    5. The answers sync back into Altegio<\/strong>, updating the client card or appointment, and generate a PDF of the completed form.<\/li>\n<\/ol>\n\n\n\n

      Because form fields map to the client’s Altegio profile, the full questionnaire is only shown on a client’s first visit<\/strong>. On repeat visits, the profile section is skipped automatically, so returning clients aren’t asked for details you already have.<\/p>\n\n\n\n

      Before you start<\/strong><\/h2>\n\n\n\n

      Make sure you have:<\/p>\n\n\n\n

      • An active Altegio account<\/strong> with access to the Marketplace.<\/li>\n\n\n\n
      • Permission to connect and manage integrations for your location.<\/li>\n\n\n\n
      • An active FastSign subscription<\/strong> \u2014 the service requires a subscription to be activated before forms can be used.<\/li>\n<\/ul>\n\n\n\n

        Setting it up<\/strong><\/h2>\n\n\n\n

        Step 1 \u2014 Connect FastSign to Altegio<\/strong><\/h3>\n\n\n\n
          \n
        1. Open the Marketplace<\/strong> in your Altegio account.<\/li>\n\n\n\n
        2. Find FastSign<\/strong> (QR Forms) by Mapty.ai and open the listing.<\/li>\n\n\n\n
        3. Click Connect<\/strong> and confirm the connection for your location.<\/li>\n<\/ol>\n\n\n\n

          Once connected, the FastSign admin panel (QR Forms Admin<\/strong>) will show an Altegio connected<\/strong> status in the top bar.<\/p>\n\n\n\n

          Step 2 \u2014 Activate your subscription<\/strong><\/h3>\n\n\n\n

          If you see a “Subscription required”<\/strong> banner in the admin panel, click Subscription<\/strong> and activate a plan. The form builder and sharing features become available once the subscription is active.<\/p>\n\n\n\n

          Step 3 \u2014 Build your form<\/strong><\/h3>\n\n\n\n

          In the FastSign admin panel, open the Form builder<\/strong>. The builder works on a simple principle: fields > PDF > sync to Altegio.<\/strong> Set the following:<\/p>\n\n\n\n

          • Form title<\/strong> \u2014 the name of the form (e.g. Questionnaire \u2013 consent<\/em>).<\/li>\n\n\n\n
          • Phone region<\/strong> \u2014 the default country used for phone validation when a client enters a number without a country code (e.g. US<\/em>).<\/li>\n\n\n\n
          • Header business info<\/strong> \u2014 your address, phone, working hours, or any details you want shown at the top of the form.<\/li>\n\n\n\n
          • Header logo<\/strong> \u2014 upload your business logo with Select file<\/strong>, or use Remove logo<\/strong> to clear it.<\/li>\n<\/ul>\n\n\n\n

            Step 4 \u2014 Configure form fields<\/strong><\/h3>\n\n\n\n

            Add the fields you want to collect. These map to your Altegio client profile and can include:<\/p>\n\n\n\n

            • First name<\/li>\n\n\n\n
            • Email<\/li>\n\n\n\n
            • Date of birth<\/li>\n\n\n\n
            • Card number<\/li>\n\n\n\n
            • Gender<\/li>\n\n\n\n
            • Middle name<\/li>\n\n\n\n
            • Comment<\/li>\n\n\n\n
            • Priority<\/li>\n<\/ul>\n\n\n\n

              For each field you can:<\/p>\n\n\n\n

              • Map it to the matching Altegio<\/strong> client-card field so answers sync correctly.<\/li>\n\n\n\n
              • Use Copy<\/strong> to duplicate a field or Delete<\/strong> to remove one.<\/li>\n<\/ul>\n\n\n\n

                Tip:<\/strong> Keep first-visit forms short. Since the profile section is skipped on repeat visits, put only the essentials in the profile fields and use separate consent or questionnaire fields for anything appointment-specific.<\/p>\n\n\n\n

                Step 5 \u2014 Share the form<\/strong><\/h3>\n\n\n\n

                Once your form is ready, share it with clients in whichever way suits your workflow:<\/p>\n\n\n\n

                • Direct link<\/strong> \u2014 copy the form link and send it to the client.<\/li>\n\n\n\n

                      Do clients need to install anything?<\/strong>
                      No. Forms open in a web browser from a link or QR code, so clients can complete them on any phone.<\/p>\n\n\n\n

                      Will returning clients have to fill in everything again?<\/strong>
                      No. Profile fields are only shown on the first visit; on repeat visits that section is skipped automatically.<\/p>\n\n\n\n

                      Can I send forms over WhatsApp?<\/strong>
                      Yes. FastSign can add the form link to the client’s notes so it can be sent automatically, including via WhatsApp.<\/p>\n\n\n\n

                      Why can’t I access the form builder?<\/strong>
                      An active subscription is required. If you see a “Subscription required” banner, activate a plan from the Subscription<\/strong> button in the admin panel.<\/p>\n\n\n\n

                      Where do submitted answers go?<\/strong>
