In large hospital systems the offer is usually sent by software. In most clinics, studios and salons the team does it by hand: it saves the request, then calls or messages the client when a matching slot frees up. If you are a patient, ask reception to add you and answer quickly when they get in touch. If you run a practice, check one thing today: where do your waiting requests live now – in a shared list next to the calendar, or on sticky notes?

TL;DR:

A waitlist entry matches a person to a specific service, team member and time window, so only relevant openings lead to an offer.

The often-quoted “36 days earlier” comes from ten high-performing US health systems with automated waitlists. It is the median by which accepted offers moved appointments forward, not a result any clinic can expect.

Fairness, speed and how often you contact the same person are house rules you set, so write them down before you start.

At a clinic, waitlist requests can reveal health information, which is special-category data under Article 9 GDPR. Record only what you need to book.

In Altegio the waiting list is part of the Appointment Calendar: the team saves requests and books a freed slot from a request in a few clicks. Offers are not sent automatically.

What is an appointment waitlist and how does it work?

A waitlist and a “call me if anything comes up” list are not quite the same thing. A proper waitlist entry is specific: the service or visit type, the team member or clinician if the person cares who it is, and the dates and times they could actually attend. A general “next available” list ignores all of that and alerts everyone. The more specific the entry, the fewer pointless calls when a slot opens.

When a cancellation happens, someone – a person or a system – checks who on the list fits the slot and makes an offer. There are two common ways to sequence offers:

One at a time: the person who has waited longest and fits the slot gets the offer, with a short window to reply before it moves on to the next person. This is fairer, but slower.

Small batches: a few eligible people are contacted at once, and the first to accept gets the slot. This is faster, but the longest wait doesn’t always win.

Automated systems do this with text messages and a reply deadline. A manual waitlist follows the same logic, except that a receptionist makes the calls in the order your house rules set.

Pro tip: Ask everyone on the list which channel they actually check – a phone call, SMS or a messenger. An offer that sits unread is a slot that stays empty.

What the “36 days” figure actually measured

The “36 days earlier” figure comes from a multisite, mixed-methods study of automated waitlists published in the Journal of Medical Internet Research in 2026 (Woodcock, Profeta and Siegel). The researchers surveyed 127 US health systems, 90 of which reported data on their automated waitlist, and studied 10 of them in depth. The participants were academic health systems, children’s hospitals and cancer centres.

Here is what the main numbers mean:

36 days: in the ten best-performing health systems, accepted waitlist offers brought appointments forward by a median of 36 calendar days (interquartile range 30–44) from the original appointment date. The figure describes people who accepted an offer, not everyone on the list.

Fill rate: in those ten systems, 38.8% of slots offered through the waitlist were filled. The median across all systems that reported data was 25%.

Acceptance: in the same ten systems, 14.4% of offers were accepted, so most offers were declined or went unanswered.

Missed appointments: 3.1% for appointments made through the waitlist, against 6.6% for all appointments.

Read these figures with care. The health systems reported the data themselves, the authors say the study did not set out to prove that the waitlist caused better access, and they did not examine what happened to the original appointments. Large US hospital systems with long queues are also very different from a three-room clinic in Lisbon or a physiotherapy practice in Kraków. What a smaller business can borrow is the way the best performers set their rules, covered below.

How do patients and clients join a waitlist?

Ask where you would normally book: at reception, by phone, or in the provider’s app or booking page if it has a waiting-list option. Many smaller practices don’t offer self-service sign-up, so a short call is often the quickest route.

To make your entry useful, give:

The service or the reason for the visit.

The team member or clinician you want, if it matters to you.

The dates and times you can realistically attend, not just “anytime”.

The contact channel you check most often.

When an offer comes, reply quickly – otherwise the slot may go to the next person. If you already hold a later appointment, ask whether it will be cancelled when you accept the earlier one, so you don’t end up holding two or losing both. Once you have booked elsewhere, ask to be taken off the list.

How to run a waitlist well: rules worth setting

Running a waitlist well is mostly a matter of rules, and most of them work just as well when your team makes the calls by hand. The best-performing systems in the study agreed on a few points: offers went out by text, patients could opt out of offers, an offered slot stayed open for normal booking while the offer was pending, and offers respected each clinician’s session limits. Seven of the ten made an offer only when it moved the appointment at least 3 days earlier, and the typical cap was one offer per patient per day.

Turned into house rules for a clinic or studio, that means:

Set a minimum improvement: contact someone only when the opening is meaningfully earlier than the appointment they already have.

Contact each person no more than once a day, and stop after a few declined offers unless they ask to stay on the list.

Keep the freed slot open for normal booking while you are calling. If a new client books it first, the slot is filled anyway.

Decide whether you call one person at a time or a small group, and whether the earlier appointment simply replaces the original one or needs a check by the team first.

Let anyone leave the list with a single reply.

Track a few numbers from the start: how many cancelled slots you refill from the list, how quickly, how many refilled appointments end in a no-show, and how much reception time the calls take. Count by hand for the first month to set a baseline, so you know whether the list is really helping.

Pro tip: Pilot the waitlist on one busy service or one fully booked team member before you open it for everything. It is easier to widen a list that works than to calm a queue of people who were all promised a call.

Health data on a clinic waitlist: what the GDPR asks

For a clinic, a waitlist holds health information more often than it seems. Under Article 4(15) GDPR (Regulation 2016/679), data concerning health covers personal data related to a person’s physical or mental health, including the provision of health care services, that reveals information about their health status. A name and phone number next to “oncology follow-up” or “fertility consultation” can therefore already count as health data.

Article 9(1) prohibits processing health data unless one of the exceptions in Article 9(2) applies. Among them are explicit consent for specified purposes (point a) and processing necessary for medical diagnosis or the provision of health care or treatment, on the basis of Union or Member State law or under a contract with a health professional (point h). Point (h) also requires the professional-secrecy conditions of Article 9(3). Article 9(4) lets each country add its own conditions for health data, so national rules differ.

Practical steps for the waitlist itself:

Record only what the team needs to book the right slot – the service, the preferred team member and times. This is data minimisation under Article 5(1)(c), so keep symptoms and diagnoses out of the comment field.

Limit access to the list to the people who schedule appointments.

Delete requests you no longer need instead of keeping years of expired entries.

Ask your data protection officer or your national data protection authority which legal basis and retention period apply to you.

How the waiting list works in Altegio

Altegio has a waiting list built into the Appointment Calendar (Digital Schedule), where your team already works with appointments. It is a reception tool: the team records the request and, when a slot frees up, turns it into an appointment. Here is how it works, according to the Altegio help centre:

Turn it on. Go to Settings > Digital schedule > Digital schedule settings > Appointments and visits and tick Show waiting list. Team members also need the Show waiting list permission in the Appointment Calendar block of access rights (Team > Team members list > Access).

Create a request. Open the Waiting list tile in the left panel under the calendar and click Create. Enter the client’s name and phone (both required), at least one service, optionally one or more team members, and one or more preferred dates and times, or tick Any time during the day. A comment carries over to the appointment.

Find the right request when someone cancels. Search by name or phone, sort by Upcoming, and filter by status (Active, Expired, Closed) and by date, for example the day open in the calendar.

Book from the request. Expand the card, click Book and pick the free slot in the calendar. The appointment window opens prefilled with the client, the services and the comment. Nothing is created until you click Save, and then the request moves to Closed.

Adding someone to the waiting list doesn’t send them anything – it is an internal record, and contacting the client about a free slot is up to your team. Once you book from the request, the appointment goes through the normal flow, so the client receives the usual appointment notifications you have set up.

What the waiting list doesn’t do yet: clients can’t add themselves through online booking, there are no automations or triggers (nothing notifies a client when a slot opens), there is no separate waiting-list report, and requests aren’t stored in the client’s appointment history. Group and package services can’t be added to a request, and the request form doesn’t check team availability – you choose a real free slot when you click Book. So plan reception time for the calls, and if you want to see how many cancellations the list has refilled, set the status filter to Closed and the date filter to All.

Two details support the data-protection steps above: without the Show waiting list permission a team member doesn’t see the list at all, and a role without access to phone numbers sees them masked.

The waiting list sits next to online booking rather than replacing it: clients book open times themselves, and the waiting list holds requests for times that are full. To make late cancellations less likely in the first place, you can ask for a deposit at online booking through your connected payment provider – see payment policies and no-show protection.

Altegio is built for appointment-based businesses – clinics, beauty and wellness, fitness, auto services and more. You can try the waiting list and the rest of the online scheduling software in a 7-day free trial. The price depends on the number of team members and the billing period.

When is a waitlist worth it?

A waitlist earns its place when demand is higher than capacity: popular team members are booked weeks ahead, cancellations happen regularly, and there are people who would gladly come in earlier. For a quiet service with free slots every day, it adds calls without adding appointments, and online booking does the job better.

Signs that it is time to start one: clients ask “can you call me if something opens up?” several times a week, one team member’s calendar is full far ahead while others have gaps, and cancelled slots regularly stay empty. A long waiting list for one service or team member is also a useful hiring signal.

For patients, a place on a list guarantees nothing. How fast it moves depends on how many people cancel and how the provider runs the list, so keep your existing appointment until the new one is confirmed.

FAQ

Does a waitlist notify clients automatically?

It depends on the system. Many large US health systems use automated waitlists that send offers by text. The Altegio waiting list has no automatic notifications: your team contacts the client, and the usual appointment notifications go out once the appointment is booked.

Can clients add themselves to the Altegio waiting list?

Not at the moment. Requests are created by the team in the Appointment Calendar. Clients can still book any free slot themselves through online booking.

How long does it take to get an offer from a waitlist?

It depends on demand and on how often people cancel. In the multisite study above, accepted offers moved appointments a median of 36 days earlier in ten high-performing US health systems, but most offers there were not accepted, and a smaller practice should not expect the same result.

What should a clinic write in a waiting-list request?

The service, the preferred team member, the dates and times and a contact number – enough to book the right slot. Leave symptoms and diagnoses out of the comment, because a waiting-list request is a scheduling record, not a medical one.

Read next

Protect your most expensive slots: payment policies, card guarantee and Adyen

A salon booking system: how to choose

Why a salon needs a booking system: 5 numbers

Where Beauty Salons Lose Bookings: 72,000 Client Messages by Hour (2026)

This article is general information, not legal advice.
